How I'd reduce shoulder injuries in CrossFit

After I my last article and video about shoulder injuries in CrossFit, a box owner left a fair comment: “So what would you do about it?”
I don’t want to be someone who points at problems without offering solutions.
So here it is — not as a CrossFit coach, but as a strength and conditioning coach with eighteen years’ experience, five of them spent reconditioning shoulders. I’ve also dislocated my own shoulder six times, once while Olympic weightlifting.
The community can do what it believes is right. But if I opened a box tomorrow, here’s what I’d put in place to reduce shoulder injuries in CrossFit.
Manage the rig: grips and kipping
First, I’d ban hand grips for anyone below Rx. Grip strength is closely tied to rotator cuff function — when I worked in elite swimming we tested it each morning as a read on shoulder readiness. Grip is a rate limiter: when the cuff fatigues, grip drops, and that naturally caps the load going through the shoulder. Grips bypass that safeguard and keep people on the rig longer than they’ve earned. Worried about torn callouses? Spend more time on the bar without grips and build tougher hands.
Next, earn the right to kip. Kipping is a legitimate, efficient skill, and elite athletes kip because they’ve built the strict strength underneath it. What I’m against is kipping before the shoulder can control the strict range it’s skipping over. A kip uses momentum to escape the dead hang — the hardest part — then drops you straight back into that same weak bottom position under bodyweight, asking the shoulder to stabilise where it’s least able to.
I wouldn’t set an arbitrary number of strict reps, though; that’s just gatekeeping. I’d manage volume instead — how much kipping someone does relative to the strict strength they currently have. Two strict pull-ups means a lower kipping ceiling than fifteen, and that ceiling rises as strict strength does. Underneath everything, strict strength work would run permanently for everyone, not as a box you tick once but as ongoing infrastructure.
Coach the quality, not just the clock
I wouldn’t celebrate poor-quality movement. I’m all for encouragement and progression, but I wouldn’t clap someone grinding out hideous reps for a number on the whiteboard. When the system is done, it’s done — pushing past that just forces the brain to find a less efficient, riskier pattern. Yes, 1RMs don’t always look pretty; that’s exactly why we don’t do them often. I’m thinking of 46-year-old Derek, chicken-winging his way to an arbitrary number of muscle-ups.
I’d also teach auto-regulation and redefine what a good workout looks like. Auto-regulation means adjusting volume and intensity to your readiness that day — “this is what my system can do today.” In a competitive room that’s hard: ego, comparison and the pull of the prescribed reps all get in the way. People are also arriving in all sorts of states — poor sleep, stress, a redundancy that morning. All stress is cumulative; training doesn’t sit in its own bucket. I’d make dialling back acceptable, and make sure members knew why, so they stay in the game long term.
Educate coaches and watch the load
How well does the average coach understand the shoulder? I won’t presume the answer, but with a known injury risk it’s a fair question. I’d invest in training and education for my coaching team so they could spot early signs and guide members well.
I’d also pay attention to the acute-to-chronic workload ratio — keeping an eye on shoulder load across the last 7 days versus the last 28. The aim is to avoid spiking volume, because sharp jumps up or down are a reliable way to upset a shoulder. That’s managed through pre-planned programming and periodisation. It’s not easy in a box, where attendance is variable and people train elsewhere too — but I can control my own programming, and I’d publish workouts in advance so members could make informed decisions about their own loading.
Build the shoulder in, not just around
I’d micro-dose shoulder health: ten minutes before or after a session of minimal-effective-dose reconditioning work, plus some thoracic spine mobility. Small investment, big return.
And I’d program far more horizontal pulling. CrossFit is heavily anterior-dominant, and shoulders like balance; load one side constantly and the imbalance shows up as injury risk over time. Horizontal pulling is what’s usually missing.
It builds strength and mobility — many CrossFitters genuinely struggle with shoulder extension (ask them to crab walk and watch). That missing extension is also where a lot of muscle-up transitions fall apart.
Rows and posterior deltoid work would feature frequently, and I think it would make a real difference. Horizontal pulling also trains the subscapularis, which matters for keeping the shoulder healthy.
Know the numbers, build the wisdom
You can’t manage risk you haven’t measured, so I’d run a simple audit on every member — a few basic strength and range markers, a few minutes with kit already in the gym. Not to diagnose anything, just to have a number today to compare against in three, six and twelve months. A quiet drop in rotation strength, or one side drifting from the other, shows up in the numbers well before it shows up as pain.
Finally, wisdom — using knowledge and experience to make good decisions. I’d build a culture where questions are welcomed and knowledge is shared. The highest expression of coaching is helping someone become able to make good decisions for themselves: less reliant on feedback, more aware of what they need.
My box might not look like yours, but this is what I’d do.
