3 Reasons your shoulder rehab didn't work (and what to do about it)

Here are three big reasons your shoulder rehab may not have worked in the past and how to fix them.



1. You Weren’t Strong Enough


At Dynamic Shoulders, we often say: “strength is armour.” Why? Because resilience is built through strength, coordination, control, and power. If your body isn’t strong enough to meet the demands placed on it, it becomes far more vulnerable to injury and far less capable of recovering from one.


When our founder, Tim Stevenson, worked in elite swimming, the number one priority of strength and conditioning wasn’t performance. It was injury prevention. Keeping athletes in the water came first because injured athletes can't win medals. The getting faster, came second.


The same principle applies to rehab:

  • Get stronger. Lower your risk of reinjury.

  • Get stronger. Improve your recovery.


Unfortunately, many standard rehab programmes stop far short of what’s needed to truly rebuild strength particularly for active and athletic people who want to return to sport or the gym. Done properly, strength training not only helps you heal, it moves you off the knife edge of vulnerability and into a place of confidence and resilience.


2. Lack of Adherence

Here’s the truth: exercise-based rehab works. It’s one of the most evidence-based tools we have for musculoskeletal pain. However, that said, it only works if you actually do the rehab you've been given.


Studies show that non-adherence rates to physical therapy can be as high as 70%. That means a huge number of people never give rehab a fair chance.


So why do people stop? Two common reasons:


Pain During Exercises. Pain doesn’t always mean harm—it’s often part of the process. But without the right guidance, pain can feel like a red flag that causes people to quit. That’s why we believe in a coaching approach. Rehab shouldn’t just be a few exercises names scribbled on a piece of paper. It should be a guided process where you can ask questions, adjust, and move forward with confidence.


Low Confidence in the Programme. If a rehab plan looks generic, rushed, or disconnected from your goals, you’re less likely to stick with it. People don’t do rehab because they love i, They do it because they want to get back to training, sport, and life.


The solution? Rehab should look and feel like training. Proper programme design, clear explanations of “why,” and modern delivery methods all build confidence, and better adherence.


3. Static Programmes Without Progression

Here’s another common problem: rehab programmes that don’t progress. Being told to repeat the same three exercises, with the same reps and sets, for weeks on end is not only boring; it’s ineffective. Strength and conditioning principles show us that adaptation requires progressive overload.


Without progression, you’re just maintaining the same level of weakness that contributed to your injury in the first place.


A better approach:

  • Adjust reps, sets, load, and tempo weekly.

  • Create fresh challenges to keep you engaged.

  • Progressively build strength so pain decreases and function improves.


This creates a positive feedback loop: strength leads to less pain → less pain leads to more consistency → more consistency leads to greater results.


The Bottom Line

If your past rehab didn’t work, it’s not entirely your fault. Yes, you have to put in the work, but it’s also possible that you were given a rehab plan that didn’t set you up for success.

  • Demand programmes that treat you like an active individual, not just a patient.
